Community Resource Navigation
Connecting your family with the resources and support you need.
Service Description
Finding the right community resources can be overwhelming, especially when you're already managing the responsibilities of caring for an aging parent. Many valuable programs and services are available, but knowing where to start—or even what to ask for—can be challenging. That's where I can help. During this consultation, we'll discuss your loved one's needs and identify local, state, and national resources that may provide additional support. Depending on your family's situation, we may explore options such as transportation services, meal programs, home care, caregiver support, financial assistance programs, medication assistance, durable medical equipment resources, senior centers, veterans' benefits, housing resources, and other community services. Rather than spending hours searching online or making countless phone calls, you'll receive guidance toward resources that are most relevant to your family's unique circumstances. I'll also help you understand eligibility requirements, provide information on how to access available services, and recommend practical next steps. My goal is to save you time, reduce frustration, and connect you with trusted resources that can improve your loved one's quality of life while easing the burden on family caregivers. Please Note: This service provides education, resource navigation, and guidance. Availability, eligibility, and acceptance into community programs are determined by the individual organizations.
