The Concierge Navigator Membership is designed for families who want comprehensive, ongoing support from a trusted Registered Nurse and healthcare navigator. Whether you're caring for a loved one with multiple chronic conditions, coordinating care among numerous specialists, managing frequent hospitalizations, or simply want the reassurance of having an experienced healthcare professional by your side, this membership provides the highest level of personalized guidance available through Aging Parent Resource Hub. As a Concierge Navigator member, you'll receive everything included in the Family Navigator Membership, along with enhanced access and premium support tailored to your family's unique healthcare journey. You'll have four personalized 60-minute consultations each month, allowing us to address concerns as they arise, prepare for upcoming appointments, review treatment plans, coordinate care, and adjust strategies as your loved one's needs evolve. This membership is ideal for families navigating complex medical conditions, dementia or Alzheimer's disease, multiple specialists, transitions between hospitals and rehabilitation facilities, home health services, hospice or palliative care, or long-distance caregiving. Rather than facing these challenges alone, you'll have an experienced healthcare partner to help you understand options, organize information, anticipate potential obstacles, and develop practical solutions. Your Concierge membership also includes priority scheduling, giving you faster access when unexpected healthcare situations arise. You'll receive ongoing care coordination guidance, assistance preparing for important medical appointments, personalized action plans following each consultation, and support identifying community resources, insurance benefits, financial assistance programs, transportation options, and other services that can improve your loved one's quality of life. As a premium member, you'll also have access to direct email support for non-urgent questions between scheduled consultations, providing added peace of mind when you need clarification or guidance. For families who desire even greater advocacy, Medical Appointment Advocacy may be added to accompany your loved one to important appointments, either virtually or in person when available.
